Caregiver in-service training log

Track one caregiver’s initial and annual in-service training for the year: date, topic, format, hours, instructor, score and sign-off, with totals to compare against your state’s requirement.

How to use this training log

  1. Start one log per caregiver per year, and write your state’s required initial and annual hours at the top.
  2. Record orientation and initial training separately from annual in-service, since many states count them differently.
  3. After each session, fill in the topic, format, hours and score, and have the caregiver initial the row.
  4. At year end, total the hours, have a supervisor sign, and keep the log in the caregiver’s personnel file.

Required topics and hours vary. See caregiver training requirements by state.

Frequently asked questions

A record of the training each caregiver completes after hire: the date, topic, format, hours, who delivered it and the result. Licensing surveyors commonly ask to see it for each caregiver.

It depends on your state and the type of license. Some states set an annual minimum for non-medical aides and others leave it to the agency. See our guide to caregiver training requirements by state, and confirm with your state’s regulation.

Often, yes, but some states limit what can be done online or require certain topics in person. Record the format on the log so you can show it either way.
